Based on medical record review and interview the facility failed to ensure that 4 of 4 Medicare patients (#3, #8, #9 and #24) received the Important Message from Medicare (IMM) resulting in the potential for patients to be uninformed regarding their Medicare patient rights.
Findings include:
On 10/6/14 at approximately 1115, during medical record review, it was revealed that patient #4's medical record failed to have the required IMM. Staff A confirmed that the medical record didn't have the IMM available for review.
On 10/6/14 at approximately 1430, during medical record review it was revealed that patient #8's medical record failed to have the required IMM. Staff A confirmed that the medical record didn't have the IMM available for review.
On 10/6/14 at approximately 1530, during medical record review, it was revealed that patient #9's medical record failed to have the required IMM. Staff A confirmed that the medical record didn't have the IMM available for review.
On 10/7/14 at approximately 1100 during medical record review it was revealed that patient #24's medical record failed to have the required IMM. Staff A confirmed that the medical record didn't have the IMM available for review.
On 10/7/14 at approximately 1200 during an interview with staff A when queried about if the missing IMM's from the patient's medical records could be found, she replied, "No, I couldn't find them. I checked with financing and they don't have them either."
