Based on document review and interview, the facility's licensure validation procedure failed to ensure that all hospital nursing personnel have a valid and current license for one of eleven employee files reviewed during the survey.

Findings:

-Human Resources (HR) department records for eleven nursing personnel were reviewed on 7/15/15 from 9 AM to 12 PM. The department file of staff #9, employed at the facility as a Licensed Practical Nurse since 3/9/2015, lacked a valid and current New York State Practical Nurse License.

-The Patient Safety Officer/Director of Clinical Risk Management verified on interview on 7/15/15 at 11:30 AM that the employee file lacked a copy of the New York State Registration form for licensed practical nurse. On-line verification of professional licensure for this employee by the hospital's representative revealed "no results."
