Based on observations made during the survey of the facility while accompanied by the hospital's CEO and Director of Plant Ops it was observed that although it appears the facility had accomplished some of the requirements regarding the required 1 hour fire-rated walls at the MED GAS STORAGE ROOM, the facility still failed to provide the following:
A. Fire-rated sealant continuously along the bottom of the rated walls (both sides rated walls are required to be sealed).
B. Stenciling was not provided (above ceiling) on "both" sides of rated walls indicating the description of the rated wall assembly.

Based on observations made during the survey of the facility while accompanied by the hospital's CEO and Director of Plant Ops it was observed that although it appears the facility had accomplished some of the requirements regarding the required 1 hour fire-rated smoke barrier walls, the facility still failed to provide the following:
A. Throughout the facility there were multiple unsealed, or improperly sealed penetrations / openings.

Based on observations made during the survey of the facility while accompanied by the hospital's CEO and Director of Plant Ops it was observed that although it appears the facility had accomplished some of the requirements regarding Air Filter Changes, the facility still failed to provide any Air Filter Change logs indicating consistent, systematic compliance.
