Based upon review of the Resident Rights, medical records, Policies and Procedures, and staff interview, the hospital failed to ensure for 2 of 2 Swing Bed medical records (#5, #6) reviewed, the Resident Rights included the patient's right to for privacy and confidentiality. Findings:

Review of the medical records for patients #5 and #6 revealed the list of Resident Rights failed to identify the patient had the right to privacy and confidentiality. Review of the Swing Bed Policies and Procedures revealed the patient's right to privacy and confidentiality failed to be identified.

Interview with S5 RN (Registered Nurse) Case Manager on 10/3/14 at 1:15 p.m. confirmed the patient's right to privacy and confidentiality was not identified in the policies and procedures or on the list of Patient Rights.

Interview with S5 RN (Registered Nurse) Case Manager on 10/3/14 at 1:15 p.m. confirmed the patient's right to retain and use personal possessions were not identified in the policies and procedures or on the list of Patient Rights.

Based on review of medical records, Policies and Procedure, and staff interview, the hospital failed to ensure that 2 of 2 Swing Bed medical records (#5, #6) reviewed, included the patient's right for married couples. Findings:

Review of the medical records for patients #5 and #6 revealed the list of Resident Rights failed to identify the patient 's right for married couples to share a room with his or her spouse when married patients lived in the same facility and both spouses consent to the arrangement. Review of the Swing Bed Policies and Procedures revealed the patient's right for married couples living arrangements failed to be identified.
