The fire alarm system did not comply with NFPA 72 as determined by:
(a) based on observation during the afternoon of April 3, a fire alarm manual pull station was not installed in the path of egresss near the building entrance/exit adjacent to the business office suite.
(b) based on review of documentation from the licensed fire alarm contractor inspection records during the afternoon of April 3, there was one smoke detector that required replacement.

The fire sprinkler system did not comply with NFPA 13 at determined by:
(a) based on observation during the afternoon of April 2, the two fire sprinklers installed in the conference room at K-3 were too far from one of the walls to provide coverage in compliance with NFPA 13.
(b) based on observation during the afternoon of April 3, the elecrical closet at the first floor main corridor (outside the Wound Care Department) was not protected by the fire sprinkler system.
(c) based on observation during the afternoon of April 3, the sprinklers in the Imaging Department file room were obstructed by the tops of rolling files installed in the room.
