Based on record review and interview, the hospital failed to ensure that each patient (or when appropriate, the patient's representative) was informed of their rights, in advance of furnishing or discontinuing patient care whenever possible as evidenced by failing to have evidence that 5 (Pt# 1, 2, 3, 4, 5) of 5 patients (or representatives) reviewed for patient rights were informed of their patient rights.
Findings:
Patient #1
Review of Patient #1's medical record revealed an admit date of 05/22/2022 and a discharge date of 05/23/2022. Further review failed to reveal documented evidence that the patient/representative was notified of the patient's rights prior to furnishing care.
Patient #2
Review of Patient #2's medical record revealed an admit date of 05/03/2022 and a discharge date of 05/05/2022. Further review failed to reveal documented evidence that the patient/representative was notified of the patient's rights prior to furnishing care.
Patient #3
Review of Patient #3's medical record revealed an admit date of 05/19/2022 and a discharge date of 05/27/2022. Further review failed to reveal documented evidence that the patient/representative was notified of the patient's rights prior to furnishing care.
Patient #4
Review of Patient #4's medical record revealed the current patient was admitted on 05/28/2022. Further review failed to reveal documented evidence that the patient/representative was notified of the patient's rights prior to furnishing care.
Patient #5
Review of Patient #5's medical record revealed an admit date of 05/28/2022 and a discharge date of 05/31/2022. Further review failed to reveal documented evidence that the patient/representative was notified of the patient's rights prior to furnishing care.
On 05/31/2022 - 06/01/2022 S5DIR reviewed the medical records for Patients #1, 2, 3, 4, and 5. In an interview on 06/01/2022 at 3:39 p.m. S5DIR verified there was no documented evidence that the patient/representative was notified of the patient's rights prior to furnishing care.
