Based on medical record review and interviews, the facility failed to infrequently use verbal orders on 10 (#1-#10) of 10 sampled closed patient medical records. This practice does not ensure the physician's care is individualized.
Findings include:
Review of the 10 (#1-#10) sampled closed records on 08/08/11 at 11:00 a.m. revealed the facility utilized the same standing orders for all patients.
This information was confirmed by the Psychiatric Intake Nurse and the Risk Manager during interviews on that date and time.
