Based on observation and interview, it was determined the facility failed to maintain exit access accessible at all times in one instance within the facility.
Findings include:
Observation of means of egress on November 2, 2012, at 11:30 AM revealed the electrical room, located at the east end of the facility, was using a hasp and padlock to secure the door. This door was the only exit from the room and could not be opened from the inside when locked.
Interview with facility representative #1 on November 2, 2012, at 11:31 AM confirmed the door could not be opened from the inside.
