Based on review of clinical record and interview it was determined the Facility failed to document meal percentages for seven (#1, #3, #7, #8, #10, #11 and #13) of 15 (#1-#15) patients. Failure to document patient's meal percentages prevented staff from assuring the nutritional needs of the patients were being met and was likely to affect all patients admitted to the Facility. The failed practice affected Patients #1, #3, #7, #8, #10, #11 and #13 on 02/04/15. Findings follow:

A. Review of Patient #1's clinical record revealed meal percentages were not documented for breakfast and lunch on 01/27/15, dinner on 01/30/15, lunch on 01/31/15, all three meals on 02/01/15 and dinner on 02/02/15.

B. Review of Patient #3's clinical record revealed meal percentages were not documented for lunch and dinner on 01/31/15 and lunch on 02/01/15.

C. Review of Patient #7's clinical record revealed meal percentages were not documented for lunch on 01/24/15, breakfast and lunch on 01/25/15 through 01/28/15, and all three meals on 01/31/15 and 02/01/15.

D. Review of Patient #8's clinical record revealed meal percentages were not documented for all three meals on 01/31/15 and 02/01/15.

E. Review of Patient #10's clinical record revealed meal percentages were not documented for all three meals on 02/01/15.

F. Review of Patient #11's clinical record revealed meal percentages were not documented for all three meals on 02/01/15.

G. Review of Patient #13's clinical record revealed meal percentages were not documented for all three meals on 01/27/15, dinner on 01/29/15, and breakfast and lunch on 02/02/15.

H. The findings in A, C and G were verified by the Chief Nursing Officer at 1120 on 02/04/15.

I. During an interview with the Risk Manager at 1140 on 02/04/15 she stated the staff responsible for monitoring the patient at meals was the staff member responsible for documenting the meal percentages. The findings, in B, D, E, and F were verified during the above interview.
