Facility failed to maintain doors in accordance with the 2000 edition of the Life Safety Code, NFPA (National Fire Protection Association). Findings included the following:

Surveyor observed between 9 and 10 am on 7/15/15 that 2 corridor doors in the Outpatient area were blocked open and in the pharmacy the latching arrangement on the corridor door required 2 actions (dead bolt separate from door latch) to unlock the corridor door.

Facility failed to maintain door to hazardous area in accordance with the 2000 edition of NFPA 101. Findings included the following:

Surveyor observed between 9 and 10 am on 7/15/15 that on the 2nd floor a soilded utility room did not have self closing device on door.

Facility failed to maintain exit lighting in accordance with the 2000 edition of NFPA 101. Findings included the following:

Surveyor observed between 9 and 10 am on 7/15/15 that in the kitchen near the coolers an exit sign light was not luminated.

Facility failed to maintain fire alarm system in accoradance with the 1999 edition of the National Fire alarm Code, NFPA 72. Findings included the following:

Surveyor observed between 9 and 10 am on 7/15/15 that on the 3rd floor mechanical room and the 1st floor electrical closet near MRI, life safety panelboards did not have red color identifying the fire alarm control circuit.

Facility failed to maintain kitchen cooking equipment protection in accordance with NFPA 10, Portable Fire Extinguishers. Findings included the following:

Surveyor observed between 9 and 10 am on 7/15/15 that in the kitchen, the class K portable extinguisher did not have the required placard reading: "fire protection system shall be activated prior to using fire extinguisher."

Facility faled to maintain electrical panelboard labeling in accordance with the National Electrical Code, NFPA 70. Findings included the following:

Surveyor observed between 9 and 10 am on 7/15 15 that in the kitchen mechanical room, the emergency electrical panel boards did not have complete identification. The panels for the essential electrical distribution from the alternate power source need to be clearly labeled to indicate the branches that these panels serve. These branch panels labels are required to read; "LIFE SAFETY", "CRITICAL" and "EQUIPMENT" [NFPA 70, 1996: 110-22, 700-9(a)]

Surveyor observed between 9 an 10 am on 7/15/15 open jboxes on the 3rd floor above the ceiling near cross corridor smoke barrier, on the first floor in the kitchen on exhaust hood, panelboard with latch missing on front cover in mechanical room, and unlcocked panel in public corridor near MRI.
