Based on a tour of the facility, the facility failed to provide a preventive maintenance program to ensure that the premises were clean and orderly.
Findings were:
During a tour of the obstetrical area on 9-8-10, 1 of 1 mattress in the labor area contained three rips/tears in the vinyl covering, each approximately 1"- 1 1/2" inches in length.
During a tour of the obstetrical area on 9-8-10, 1 of 2 surgical scrub sinks did not provide hot water despite allowing the faucet to run for >8 minutes.
The above was confirmed in a meeting with the CEO and DON on the afternoon of 9-8-10 in the administrative conference room.

Based on a tour of the facility, the facility did not have a preventive program to ensure that there is proper ventilation, lighting and temperature control in all pharmaceutical, patient care and food preparation areas.
Findings were:
During a tour of the patient care unit on 9-8-10, there was no documentation to show that the refrigerator temperature had been checked on 1 of 1 medication refrigerators and was within the acceptable range for 2 of the 8 days in the month of September 2010.

Based on a tour of the facility, the facility did not ensure that outdated, mislabeled or otherwise unusable drugs were not available for patient use.
Findings were:
During a tour of the operating room on 9-8-10, 25 of 25 vials of sterile water had expired 10-09 but were still available for patient use.
During a tour of the cardiac rehabilitation area on 9-8-10, 1 of 2 250-ml bags of intravenous dextrose 5% had expired 8-10 but was still available for patient use.
During a tour of the cardiac rehabilitation area on 9-8-10, 1 of 2 1000-ml bags of intravenous lactated ringers had expired 5-10 but was still available for patient use.
During a tour of the cardiac rehabilitation area on 9-8-10, 1 of 1 1000-ml bag of intravenous normal saline had expired 8-10 but was still available for patient use.
