Facility failed to maintain door to hazardous area in accordance with 2000edition of NFPA (National Fire Protection Association) Life Safety Code. Findings include:
Surveyor observed on 10/ 12/10, between 10:00 and 10:30 am that door to x-ray reading room, storage room in excess of 50 sq ft storing film, did not have self closing device.

Facility failied to maintain electrical systems in accordance with NFPA 99, Standard for Health Care Facilities and NFPA 70, National Electrical Code. Findings included:
Surveyor observed on 10/13/10 between 10:00 and 10:30 am that essential electrical sytem panels were not labeled properly, e.g., new panel in X-ray reading room and distribution panels in corridors, in accordance with NFPA 70, 1996: 110-22, 700-9(a) and NEC, 1996: 110-22 and 700-9(a). The panels for the essential electrical distribution from the emergency generator need to be clearly labeled to indicate the branches that these panels serve. These branch panels labels are required to read; "LIFE SAFETY", "CRITICAL" and "EQUIPMENT". In all electric panel boxes, the circuit directories need to be labeled in accordance to which circuit breakers serve what electrical devices by room name and room number or any equipment with proper name as required by NFPA 70, 1999: 384-13 and NEC, 1999: 384-13.
