The surveyor determined through observation during a walk-through with the Maintenance Director and other maintenance staff August 5, 2014 that the one-hour separation for the Elevator Hydraulic Room located in the basement did not conform to the smoke and fire resistance characteristics contained in the 1999 edition of the NFPA 101, the Life Safety Code. This deficiency, which was acknowledged by the Maintenance Director, has the potential to affect an indeterminant number of staff and visitors.
The sheetrock over the lintel (header) of the corridor door, and located within the Elevator Hydraulic Room, is ruptured; thereby, compromising the smoke and fire resistance characteristics of the room.
This does not comply with the requirements contained Section 19.3.2.1 of NFPA 101.

The surveyor determined through observation during a walk-through with the Maintenance Director and other maintenance staff August 5, 2014 that the facility did not maintain the Exit Discharge path in accordance with requirements of the 2000 edition of NFPA 101, Life Safety Code. This deficiency, which was acknowledged by the Maintenance Director and others, has the potential to affect more than 50 occupants, as many as 4 residents, as well as an indeterminant number of staff and visitors.
There is a locked gate in the path of egress for those occupants exiting from the basement on the north side of the building.
This route is a common pathway for occupants exiting the basement as well as occupants exiting the secured unit, and the locked gate precludes a route to the public way for those occupants exiting the basement.
Section 7.2.1.5 of the 2000 edition of NFPA 101 states, in part, ". . . Locks, if provided, shall not require the use of a key, a tool , or special knowledge or effort for operation from the egress side."
