Based on observation and interview, the facility failed to provide an emergency release switch for the full time magnetic locks on the Labor and Delivery Unit per the requirements of:
2012 NFPA 101, 19.2.2.2.5.2 (1), and 19.2.2.2.6
This deficiency could affect 10 occupants.
Findings include:
On 06/05/2019, during a tour of the facility from 8:00 am to 12:30 pm, the surveyor observed the Labor and Delivery Unit did not have an emergency release switch that automatically unlocked the full time magnetic locked doors in case of an emergency. This unit did have key pads that released each individual door.
A member of the maintenance staff was present when this deficiency was identified.

This deficiency could affect 10 occupants.
Findings include:
On 06/05/2019, during a tour of the facility from 08:00 am to 12:30 pm, the surveyor observed the automatic fire extinguishing discharge nozzles were not in line with the cooking appliances. Per the facility the nozzles under the kitchen hood had been moved for the purposes of maintenance and cleaning of the cooking appliances and had not been returned to the approved design alignment prior to cooking operations.
A member of the maintenance staff was present when this deficiency was identified.

Findings include:
On 06/05/2019, during a tour of the facility from 8:00 am to 12:30 pm, the surveyor observed a smoke detector dismounted from its base and hanging by the circuit wiring in the Housekeeping Room 2206 on the Second Floor.
