Based on review of facility documents, observation, and staff interview (EMP), it was determined staff working in the laboratory failed to ensure personal clothing was protected when working with potentially contaminated blood and body fluids.

Findings include:

A request was made of EMP2, EMP3 and EMP4 on March 3 and 4, 2015, for a facility policy or procedure instructing laboratory personnel on the protection of personal clothing while working in the laboratory area. No policy was provided.

Observation on March 3, 2015, at approximately 8:30 AM revealed EMP6, EMP7, EMP8, EMP9, EMP10 and OTH1 working in areas of the laboratory with potentially contaminated blood and body fluids. Further observation revealed these employees did not have their laboratory coats buttoned to protect their clothing.

Interview with EMP3, EMP4, EMP5 on March 3, 2015, at approximately 8:45 AM confirmed EMP6, EMP7, EMP8, EMP9, EMP10 and OTH1 were working in areas of the laboratory with potentially contaminated blood and body fluids, and these employees did not have their laboratory coats buttoned to protect their clothing.
