Based on observation at the time of inspection, the facility failed to maintain an exit door in an exit passageway. The required Exit door adjacent to the stairwell located on the exterior wall of the building just before the corridor connecting the hospital to the clinic failed to open.
Based on observation at the time of inspection, the facility failed to maintain the Stairwell exit doors on the third floor of the facility. The third-floor plan west stairwell door failed to latch properly. The plan east stairwell door did not have a lever. Provide lever handle type of hardware.

Based on observation at the time of inspection, the facility failed to provide proper labeling to existing disconnects. All disconnects associated with the essential system must be properly labeled.
General. Each disconnecting means shall be legibly marked to indicate its purpose unless located and arranged so the purpose is evident. The marking shall be of sufficient durability to withstand the environment involved. NFPA-70 110.22 Identification of Disconnecting Means. Identification. All boxes and enclosures (including transfer switches, generators, and power panels) for emergency circuits shall be permanently marked so they will be readily identified as a component of an emergency circuit or system. NFPA-70 700.10 Wiring, Emergency System.
Based on observation at the time of inspection, the facility failed to provide proper labeling of the emergency panels.
"Emergency panels shall be distinctively marked so that they are readily identifiable as a component of the essential electrical system." - NFPA 70, 2002: 110-22 and 700-9(a). They shall be labeled "LIFE SAFETY", "CRITICAL", or "EQUIPMENT" as applicable.
Based on observation at the time of inspection, the facility failed to provide proper labeling of the emergency panels.
"Panel directories shall be labeled to legibly identify the purpose or use of each circuit." - NFPA 70, 2002: 408.4.

Based on observation at the time of inspection, the facility failed to provide a properly maintained and functional Essential system. At the time of inspection, the Transfer switch for the critical system not properly working. Facility in contact with contractor to get this work done. It is imperative that this work gets done as quickly as possible.
