Based on an observation, it was determined that a section of the facility corridor wall failed to act as the smoke barrier to meet the required 30-minute rating for the exit egress path. This deficient practice could affect patients, staff and visitors if smoke were to fill the exit corridor from the adjacent room.

Findings include:

On 06/12/2015 at 10:26 AM while accompanied by facility representatives it was determined that on the main floor, the corridor leading to laundry contained a transfer grill in the wall of the sterile processing room. This open grill does not comply with NFPA 101, section 19.3.6.1.

Based on an observation of hazardous areas the facility failed to provide properly operating latching and self-closing door hardware that provides separation between hazardous areas and exit access corridors. This deficient practice could affect patients, staff and visitors if a fire could spread without proper fire separation.

Findings include:

A. On 06/12/2015 at 10:20 AM while accompanied by facility representatives it was determined that on the main floor, Surgery Suite, soiled utility room door was not installed with latching hardware. Soiled Utility rooms are considered hazardous and shall comply with NFPA 101, section 19.3.2.1.

B. On 06/12/2015 at 11:35 AM while accompanied by facility representatives it was determined that on the main floor, Stage II recovery area contained a soiled utility room and the door to the room was not self closing. This does not comply with NFPA 101, section 19.3.2.1.
