A) (New 10/13/10): There is a pair of fire doors between the new Bed Tower and the existing Atrium Building at the east side of the construction project in the Lower Level (Grease Trap Project area). The fire doors were damaged to the point that they could not be closed to latch. Adequate interim measures to find and abate such conditions have not been implemented. The cited doors occur at a designated four hour fire separation between buildings.

There are also unsealed fire penetrations in the fire wall above the doors.

A) (New 8/19/10): The required exit stair located at the southeast corner of the Atrium Building has been compromised by construction activities.

d) The enclosure walls of the exit stair have been compromised on multiple floors, inside the stair. The drywall that is part of the fire rated enclosure has been cut and removed by contractors. The drywall was placed back in the openings and sealed with cellophane tape. The walls were not repaired to provide and maintain the required fire ratings.

10/13/10: The above item was not fully corrected in accordance with the last submitted PoC. A new hole, sealed with clear tap was observed at level 4 1/2.

Identify the U L Design Number of the wall assembly and identify the patching details used to maintain the two hour fire rating.

e) (New 1-13/10): The above stair is used as a smoking and dining location (stairs from Lower Level to 1st Floor). Multiple steps were found with food products, beverage containers and debris.

f) New 1-13/10): There is a leaking sprinkler valve at the 2nd Floor in the above stair, with a rag on the floor. The leaking valve was not detected and abated by interim measures.

B) (New 8/19/10) Adequate interim Life Safety Measures have not been implemented to monitor construction personnel, relative to occupied buildings, to find and abate life safety deficiencies.

C) (New 10/13/10): 2nd Floor Stair D - the stair door was taped so that the latchset would not function.

The surveyor observed that unattended vehicles are allowed to park under this canopy. The canopy lacks sprinkler protection in accordance with 5-13.9 (NFPA 13-1999) (provide sprinkler protection or prohibit unattended vehicles which constitute combustible storage).

03/26/10: The canopy area still has signs that indicate that Medical Center Parking is permitted under the canopy. - Corrected 10/13/10; however, the provider is unable to prevent unattended cars from being left under this canopy, in spite of striping and no parking signs. Failure to maintain adequate measures to prevent this will result in a requirement to sprinklere the canopy. Identify what interim measures will specfically be implemented to prevent re-occurance.
