Based on interview and record review the facility failed to identify a dedicated governing body that was responsible for the conduct of the Critical Access Hospital as an institution.

Employee (C)/Senior Officer was interviewed on 6/23/2010 at 9:23 a.m. and stated that the governing authority for the SMDC Health System is the governing body for all entities, and is the ultimate authority for the governance of the facilities within the SMDC Health System. Each facility has a board that serves in an advisory capacity which includes physicians, community members, and staff. This board provides a vehicle to receive community imput, oversight, and strategy for the application of new services. Each facilty has a Medical Staff Executive Committee which will approve the medical staff, and the final approval will be made by the SMDC Board.

The Pine Medical Bylaws Rules and Regulations of the Medical Staff documents that the Governing Body means the Board of Directors of St. Mary's/Duluth Clinic Health System (SMDC).
