Based on observation and confirmed by Gavin M. Latney, Director of Safety and Security, the facility failed to maintain exits that were accessible at all times.
It was observed that one of two marked exits from the waiting room area of the Emergency room was a Access -Control Egress Door that did not meet the minimum requirements for Controlled Egress doors. The doors lacked a sensor to detect an occupant approaching the door in the direction of egress travel and lacked a manual release device within five feet of the door.
It was also observed and confirmed by Mr. Latney, Director of Safety and Security, that the Controlled Access exit doors within the Emergency Room and at the admissions desk lacked manual release devices within five feet of the doors.
