RN SUPERVISION OF NURSING CARE

Based on document review the nursing manager failed to ensure the implementation of the facility policy related to staffing requirements for 9 of 28 days for October 2016.
Findings Include:
1. Review of the policy Nursing Staffing Plan, policy number II.C,107, last revised/approved 5/2016, indicated in "a. Nursing Staffing Grid" that with 24 to 30 patients, 1 RN (registered nurse) and 3 RN/LPNs (licensed practical nurses) are required and with 31 to 36 patients, 1 RN and 4 RN/LPNs are required
2. On 10/9/16, the census was 31 but nursing staffing was 4 (combination of RNs and LPNs) in which the policy requires 5 nurses for 31 to 36 patients.
3. On 10/16/16 and 10/18/16 the unit had 33 and 32 patients with only 4 nurses present in which the policy requires 5 nurses. The day shift on 10/17/16 had only 4 nurses for 32 patients in which the policy requires 5 nurses.
4. The night shifts of 10/23/16 and 10/24/16 had 32 and 33 patients with only 4 nurses scheduled and 29 patients on 10/29/16 with 3 nurses scheduled. The scheduling for those 3 shifts was not per policy.
5. The day and night shifts for 10/31/16 had 4 nurses with 31 patients, 11/3/16 and 11/4/16 had 32 patients with 4 nurses on both the day and night shifts. The scheduling for those shifts was not per policy.
