Based on interview, medical record review and physician's orders, the facility failed to ensure the provision of nursing care to all patients in the form of hemodialysis as needed per physician orders for 1 of 4 sampled patients (#1).

Findings:

Patient #1's medical record reflected that the patient was in the hospital from 10/06/19 (Sunday) to 10/17/19.

Physician orders of 10/06/19 at 7:51 PM indicated hemodialysis treatments three times a week, Monday, Wednesday and Friday, for 3 ½ hours each treatment. Thus, the first treatment was expected to take place on 10/07/19.

The Nephrologist saw the patient on Monday, 10/07/19 at 2:49 PM. He wrote, "She is willing to do....HD (hemodialysis) today." There was no evidence in the medical record of any attempt to dialyze the patient on 10/07/19, a scheduled day for treatment per physician orders.

During an interview of the Risk Manager on 12/23/19 at approximately 2:45 PM, she confirmed that the patient did not receive hemodialysis on 10/07/19, as ordered by the physician.
