Based on observation and staff interview, the facility failed to maintain the environment of the patient on units 2 North, 3 South and Nicolls Cottage in a manner that ensures safety and well being of the patients.

Findings include:

During tour of the facility units 2 North, 3 South and Nicolls Cottage, on 3/17, 3/18 and 3/21/16, it was observed that the door handles of several rooms of the psychiatric units, including the patient rooms and bathrooms had regular door handles, not the safety handles that were required for psychiatric units. This is a potential of looping hazards.

This finding was acknowledged by Staff E, the Director of Nursing Practice, who was present during the tour.
