The facility failed to provide corridor doors that would close and resist the passage of smoke. Findings include: During the survey, the following is an example of what was observed.
The dutch door which opens into the corridor from the kitchen, when the upper leaf of the door was closed this surveyor was able to see into the corridor.

19.3.6.3.6 Dutch doors shall be permitted where they conform to 19.3.6.3 In addition, both the upper leaf and lower leaf shall be equipped with a latching device, and the meeting edge of the upper and lower leaves shall be equipped with an astragal, a rabbet, or a bevel.

The facility failed to maintain the exit access per code. Findings include:
During the survey, the following is an example of what was observed:
(A) The following door had one releasing mechanism which was a deadbolt, with the thumb turn to open from the inside of the Lab Sleep Room 162. The releasing mechanism was located 57" above the finished floor.

2000 NFPA 101, 7.2.1.5.4 A latch or other fastening device on a door shall be provided with a releasing device having an obvious method of operation and that is readily operated under all lighting conditions. The releasing mechanism for any latch shall be located not less than 34 in. (86 cm), and not more than 48 in. (122 cm), above the finished floor. Doors shall be operable with not more than one releasing operation.
(B) Delayed egress locking device was observed installed on the exit door. This section of the facility was observed without an approved, supervised automatic fire detection system or an approved supervised automatic sprinkler system.

2000 NFPA 101, 7.2.1.6.1 Delayed-Egress Locks. Approved, listed, delayed-egress locks shall be permitted to be installed on doors serving low and ordinary hazard contents in buildings protected throughout by an approved, supervised automatic fire detection system in accordance with Section 9.6, or an approved, supervised automatic sprinkler system in accordance with Section 9.7, and where permitted in Chapters 12 through 42, provided that the following criteria are met.

Sprinkler coverage was observed during the survey not adequately provided. Findings include: During the survey, the following is an example of what was observed.
Bent deflector on a sprinkler in the closet of the payroll office.

1999 NFPA 25, 2-2.1.1 and 2-4.1.2 Sprinklers that are painted, corroded or damaged shall be replaced with new listed sprinklers of the same characteristics, including orifice size, thermal response, and water distribution.

The facility failed to provide approved electrical utilities. Findings include: During the survey, the following are examples of what was observed.
Two electrical panels located by Patient Room 156, one had one open blank, the second panel had four open blanks.

1999 NFPA 70, 373-4 Unused opening shall be effectively closed to afford protection substantially equivalent to that of the enclosures.
