Observations revealed that one of six required exterior exit doors is blocked against egress and is not in accordance with NFPA 101 "The Life Safety Code" 2000 edition (LSC) section 19.2.1 and 19.2.5.10. This deficient practice could prevent the occupants from exiting in a timely manor in an emergency, effecting the any patients in, staff and guests of the West wing.
Findings include:
Observations during the facility tour on August 19, 2010, between 8:45 am and 11:00 pm, revealed that the West Business Offices wing exit has been blocked to prevent use, and creates a dead end corridor due to construction. This is to prevent residents form being injured by exiting into the construction zone. This creates a 50+ foot dead end corridor in this sprinkler protected building. The facility has developed a plan and added directional signage to direct occupants to an operational exit and can create a suite of rooms by sealing the gap between the cross corridor door that closes and latches, and the temporary wall.
The Environmental Services Director (MM) verified this finding during the facility tour and with the Administrator (PW)during the exit conference.

Observations indicated that the automatic sprinkler system has been taken out of service in two areas under construction. and is no longer in accordance with NFPA 13 Standard for the Installation of Sprinkler System 1999 edition. This deficient practice may allow a fire to grow which will negatively impact all the residents, visitors and staff.
Findings include:
Observations during the facility tour on August 19, 2010, between 8:45 am and 11:00 pm, revealed that in the two areas under remodeling have had the structural supports of the building exposed and the sprinkler system has been capped. Therefore the construction type of the building is a Type II (000) and is required to be fully sprinkler protected which it no longer is.

Observations revealed that the facility staff have items that obstruct the full and instant use of the corridors in an emergency which is required by NFPA 101 "The Life Safety Code" 2000 edition (LSC) section 7.1.10.2.1. This deficient practice can slow or even prevent exiting from the wing that is obstructed, effecting the 20 residents, staff and guests of those wings.
Findings include:
Observations during the facility tour on August 19, 2010, between 8:45 am and 11:00 pm, revealed that, in the South Patient wing, a computer work station, 2 task chairs, a linen cart and 2 IV pump poles have been left unattended in and obstructing the use of this corridor.
