Based on observation, the facility failed to provide and maintain reliable emergency lighting in accordance with NFPA 101(2000) 7.8; 19.2.8
Findings:
On August 24, 2011 at 1:12 PM while on tour of the facility with facility staff, the emergency lights in the generator room failed to illuminate when tested. The emergency lights in the fire pump room and boiler room also failed to illuminate when tested. Monthly testing of emergency lights is required to insure reliable operation in the event of a power failure. Annual testing is required to insure the durability of the batteries for the required period of time as specified in NFPA 101 (2000) 19.2.9.

Based on observations, the facility failed to maintain the sprinkler system in accordance with NFPA 25.
Findings:
On August 24, 2011 at 1:30 PM while on tour of the facility with facility staff, the storage closet identified as room 1163 was observed to be unsprinkled.
Interviews with staff, at this time, revealed that a new ceiling had been installed in the room below the level of the existing sprinkler head.
Sprinklers shall be installed and maintained in accordance with NFPA 13 and NFPA 25.

Based on observation, the facility failed to install and maintain fire extinguishers in accordance with NFPA 10.
Findings:
On August 24, 2011 at 10:30 AM while on tour of the kitchen with facility staff, the "K" type fire extinguisher was observed mounted in a wall cabinet and was missing the required placard indicating the use of the extinguisher only after the fixed suppression system is activated. Installation of this placard is required to notify kitchen staff of the required procedure for extinguishing a grease or kitchen fire. NFPA 10 (2002) 4.3.2.2; A 4.3.2.2(a)
