Based on observation and staff interview, the facility allowed self-closing fire doors to be held open by an unapproved method. This practice would allow smoke to migrate into the exit corridors.
Findings are:
Observation on 8/23/23, from 10:09 am to 11:08 am revealed:
1. Three Lab doors were held open by door chocks or a chair and prevented the doors from self-closing.
2. The Pharmacy IV Room door was held open by a door chock that prevented the door from self-closing. A door chock was observed on the floor by the Main Pharmacy door.
In an interview on 8/23/23, from 10:09 am to 11:08 am, Maintenance A acknowledged the findings.

Based on observation and staff interview, the facility failed to separate hazardous areas with smoke resistive partitions. This condition would allow smoke to migrate into the exit corridors.
Findings are:
Observation on 8/23/23, from 10:01 am to 10:33 am revealed:
1. Elevator Equipment Room door did not positively latch when self-closed.
2. Business Office door did not positively latch when self-closed.
In an interview on 8/23/23, from 10:01 am to 10:33 am, Maintenance A acknowledged the findings.
