Findings include:

During the tour on 4/17/12 between 12:30 PM and 3:45 PM the following is observed:

--1) There is unsealed sheet rock seams around pipes and sheet rock that had been installed to cover exposed wood up against the smoke barrier wall, and there is a small hole in the lower corner where wires have been removed on the West side of the one smoke barrier wall.
--2) There is exposed wood against the smoke barrier wall, a gap between the heating & cooling pips, two open wire chases and other gaps around conduits and wires on the East side of the one smoke barrier wall.

Maintenance Staff A was present and acknowledged the finding.

NFPA Standard: Smoke barriers shall be continuous from an outside wall to an outside wall. Such barriers shall be continuous through all concealed spaces, such as those found above a ceiling, including interstitial spaces per NFPA 101, 8.3.2. When pipes, conduits, cables, wires, air ducts and similar building service equipment pass through smoke barriers, the space between the penetrating item and the smoke barrier shall be filled with a material that is capable of maintaining the smoke resistance of the smoke barrier or protected by an approved device that is designed for the specific purpose per 2000 NFPA 101, 8.3.6.1

During the tour on 4/17/12 between 12:30 PM and 3:45 PM the following is observed:

--1) There is no self closing device on the over 50 sq ft storage room, there are gaps around pipes and conduits on the east wall, and there is an access door through the east wall that has been screwed back onto the wall and there is a gap around the entire door. The access door, when removed, is open to the Boiler/Riser room - in the upper Loft Storage room.
--2) The under side of the Loft stairs are inside a small storage room. The stairs are plywood as well as a section of the floor that is exposed to the storage room that opens up into the Boiler/Riser room.
There is no one hour separation between the upper Loft storage room and the Boiler/Riser room.
--3) There is a small hole on the back wall and gaps around pipes on the corridor wall of the Storage room.

During the tour on 4/17/12 between 12:30 PM and 3:45 PM it is observed when the primary phone line was disconnected from the fire alarm panel - the monitoring company received the phone line fault within 3 minutes. The fire alarm sounded for a short time then went silent. The FAP (fire alarm panel) did not go into trouble mode. The alarm memory light was lit at all times surveyor was in the building.
