Based on observation, it was determined that the facility failed to provide one hour rated construction for self closing doors of storage areas.

Findings include:

1. On 11/29/11 at 9:50 AM in the presence of Staff #9, the door to Storage Room #LL2 could not close.

2. On 11/29/11 at 10:05 AM in the presence of Staff #9, in the northeast corner of the main storage room, a penetration went through the ceiling to the lab above.

2. On 11/29/11 at 11:15 AM in the presence of Staff #9, in the main medical records room, the door closer device had been removed.

Based on observation, it was determined that the facility failed to protect all gas in storage.

Findings include:

1. On 11/29/11 at 10:55 AM in the presence of Staff #9, in the HVAC storage room, a tank of refrigerant gas was unsecured.

Based on observation, it was determined that the facility failed to ensure that Alcohol Based Hand Rub dispensers (ABHR) are not installed above ignition sources.

1. In the presence of Staff #9, in Room #2245, an ABHR was installed above an electrical outlet.
