Based on observation, it was determined that the facility failed to provide one hour rated construction for self closing doors of storage areas.

Findings include:

1. On 11/29/11 at 9:50 AM in the presence of Staff #9, the door to Storage Room #LL2 could not close.

2. On 11/29/11 at 10:05 AM in the presence of Staff #9, in the northeast corner of the main storage room, a penetration went through the ceiling to the lab above.
