Based on observation and interview, the facility failed to follow standard infection practices during an injection for two of two random observations.
The findings included:
Observation on 6/16/15 at 11:25 AM on Unit I, revealed Licensed Practical Nurse (LPN) #1 preparing an injection for a patient. Further observation revealed the nurse wiped the injection site with alcohol and, prior to the injection, took off the cap of the needle with her mouth. Further observation revealed during the injection the cap of the needle remained in the nurse's mouth.
Observation on 6/16/15 at 11:30 AM on Unit I, revealed Licensed Practical Nurse (LPN) #1 preparing an injection for a patient. Further observation revealed the nurse wiped the injection site with alcohol and, prior to the injection, took off the cap of the needle with her mouth. Further observation revealed during the injection the cap of the needle remained in the nurse's mouth.
Interview with LPN #1 on 6/16/15 at 11:31 AM, in the Unit I medication room, confirmed the nurse removed the needle cap from the syringe with her mouth prior to the injections for both patients.
Interview with the Infection Control Nurse on 6/16/15 at 11:50 AM, in the conference room, confirmed the nurse failed to follow standard infection control practices for the injections.
