Based on observation and documentation the Rehabilitation Center located on Marco Island failed to provide a properly tested and maintained fire alarm system. The deficient practice would affect all patients and staff.
The findings include:
During the documentation review on 12/15/11 at 2:10 p.m. it was noted the last fire alarm system inspection and testing had not been conducted within the last twelve months. The documentation that was reviewed indicated the last inspection and testing was conducted in November 2010.

Based on a review of the facility records and interview with the Maintenance Director on 12/15/11, it was determined the facility failed to ensure the emergency back up generator was maintained as required. This renders the equipment unreliable and could result in damaged equipment, or equipment failure when there is a loss of municipal power.

During the documentation review and inspection of the emergency generator at the Urgent Care Center on Marco Island it revealed at 2:10 p.m. the facility failed to perform the annual two hour load bank test as required.
