Based on observation and interview the facility failed to clean, sanitize and maintain 3 of 3 bathrooms that were inspected.
A tour of the Adult Behavioral Health Unit was conducted on 04/14/2015 at approximately 10:30 AM with staff #1 and staff #2. During the tour the bathrooms in room ' s 523, 530 and 532 were observed to have dirty floors and were in need of cleaning.
Room 523 ' s bathroom was observed to have hair on the floor. A towel was observed lying in the floor. The bathroom did not appear clean.
The tour continued to room #530. The bathroom in room #530 was observed to have bubbled and peeling paint near the floor in the shower area. The bubbled and peeling paint near the floor in the shower area could not be properly sanitized and disinfected.
The tour continued to room #532. Room #532 ' s bathroom was observed to have hair on the floor. The area around the toilet appeared dirty. The surveyor whipped the floor in the area of the toilet with tissue paper. The results were the tissue was covered in what appeared to be lint, and a hair.
An interview with staff #1 confirmed room #523, #530 and #532 were said to be clean and ready to receive a new patient.
An interview with staff #1 and staff #2 confirmed the bathrooms in room #523 and #532 were not clean at the time of the tour. Staff #1 and staff #2 confirmed the findings of peeling paint in room #530 ' s bathroom.
