Based on a review of the Emergency Department surgery on-call lists and staff interview, it was determined that the facility failed to ensure that the physician on-call list identifies an individual physician's name for each specialty on call.
Findings include:
1. A review of the on-call list for the neuro-surgical service revealed that the name of a physician group was listed as on call from October 1, 2015 through March 31, 2016.
a. A specific surgeon's name was not listed.
2. The above was confirmed by Staff #2.
