PATIENT RIGHTS:PARTICIPATION IN CARE PLANNING

Based on clinical record review and staff interview it was determined the facility did not allow the patient and family of 1 (#1) of 9 clinical records reviewed to participate in the patient's plan of care by not informing them of the reasons for the delay in the patients surgery.

Findings Include:

A review of patient #1's clinical record revealed the patient was admitted to the facility on 11/11/10 with a left hip and left distal radius fractures. A orthopedic consult was completed on 11/12/10 at 1:43 a.m., The Physicians plan was once the patient was medically cleared, she would be taken to the operating room for an open reduction internal fixation of the left hip. The patient was medically cleared on 11/13/10 at 7:15 p.m. The patient went to surgery on 11/15/10 at 11:30 p.m. A review of the operating record revealed the patient went into the operating room at 2:05 a.m. on 11/16/10 and the operation ended at 4:10 a.m.

An interview with the Patient Safety Officer, on 5/24/11 at 3:00 p.m. after thorough review of the patients clinical record confirmed there was no documentation by Nursing and/or Physicians addressing the delay in the patients surgery.

An interview with the Chief Medical Officer (CMO) was conducted on 5/25/11 at 1:30 p.m. which revealed the CMO was surprised the surgery was not done sooner.
