Based on observations during the facility walk-through inspection with the director facility management, and life safety coordinator, on 02/13/2014, it was noted that the fire alarm annunciator panel located at the front lobby lacked functioning indicator lights. When the fire alarm system was tested during this walk-through inspection , the annunciator failed to light up the "alarm" or "silence" lights. Also when the light test button was pushed, none of the indicator lamps activated.

Based on the facility walk-through inspection and discussions with the director facilities management and life safety coordinator, on 02/12/2014, it was determined that the main fire alarm control panel did not have identification labeling, on the face of the panel, indicating the electrical panel and circuit number (NFPA 72, 4.4.1.4.2.3) .

Based on observation and conversations with the director facilities management and life safety coordinator, during the facility walk-through inspection, on 02/12/2014, it was noted that the medical gas tanks at the manifold location, were not individually secured with chains or brackets [NFPA 99, 5.1.3.3.2(7)]. The tanks were chained in groups, instead of singularly.

Based on observation and conversations with the director facilities management and life safety coordinator, during the facility walk-through inspection, on 02/12/2014, it was noted that there was no remote annunciator panel for the emergency generator, located in the facility engineering office or work space (NFPA 99, 4.4.1.1.18). The only annunciator panel was located at the PBX office.

Based on the facility walk-through inspection, with the director facilities management and life safety coordinator, on 02/13/2014, it was noted that;
a). There was no duplex electrical outlet located at the generator set location, powered from the Life Safety branch of the EES [(NFPA 99, 4.4.2.2.2.2.(5)].
b). there was not a remote annunicator panel for the generator, located at the engineering office or work area [NFPA 99, 4.4.1.1.18]. There was only a generator annunciator panel at the Skilled Nursing Unit nurse station.

Based on the facility walk-through inspection with the director facilities management and life safety code coordinator, on 02/13/2014, it was observed that the nurse call system was powered from a Life Safety branch electrical panel "ESL", circuit #4, located within an electrical closet in the Rehab. Nursing Wing. Nurse call systems shall be powered from the Critical Branch of the EES [NFPA 99, 4.4.2.2.2.3(5)].
