Based on observation and interview with facility staff , the facility failed to maintain 1 of 2 two hour barriers that separate nonconforming buildings from the hospital. In the event of a fire all 25 residents may be affected.

Findings Include:

On 01-16-2013 at approximately 11:45 a.m. observation revealed sheathing missing from the 2 hour barrier at the administration office.

Staff member M confirmed the findings at the time of discovery.

Based on observation and interview with facility staff the facility failed to ensure sprinkler piping is not used to support items in the attic. In the event of sprinkler failure all 25 residents may be affected.
