Based on observation and interviews, it was determined the facility failed to ensure personal protective equipment (PPE) was readily available for staff use and ensure staff knowledge on where to obtain the supplies. The facility failed to ensure all potentially contaminated surfaces are thoroughly cleaned and disinfected.

Findings included;

1. Tour of the 4 South Progressive Care Unit (PCU) on 01/11/2016 at 2:00 p.m. revealed a patient room with a sign designating the patient required contact and droplet precautions. Observation revealed no face shield or goggles readily available for use. Interview with Registered Nurse (J) indicated she did not know where she would obtain the splash guard or the eye protection if she needed one. The surveyor accompanied the Director of Infection Control to locate eye protection. None was found on the unit or in either of the two supply rooms for the unit.

2. Observation of the cleaning of a patient vacant room (466) on 01/11/2016 at 3:55 p.m. revealed film and debris on the over bed table in two locations. Observation was confirmed by the Director of Nursing.

01/12/2016 at 11:00 a.m. interview with the director of infection control and the chief quality officer confirmed the above findings.
