PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation, interview and record review the facility to provide care to protect patients when,

a.) (2) two out of (3) three patients were observed with Intravenous (IV) tubing and IV catheter sites that were not labeled when they had been changed or were hung, according to the facility policies and
b.) the facility failed to provide respectful care when (3) three out of (3) three patients were not offered showers. (Patients #2,3 and 8)

Findings Include:

a.) Observations made on the morning of 2/26/18, on the facility's 3rd floor telemetry unit, revealed:

Patient #3 in Room 326 with a peripheral IV catheter in the right antecubital, the transparent dressing did not have an insertion or dressing change date. An IV antibiotic Zosyn and an IV normal saline bag were hanging from an IV pole. The tubing was not labeled when they had been hung.

Patient #2 in Room 321 with an implanted port-a-catheter to the right chest, the transparent dressing did not have a written date the dressing had been changed last.

Review of the facility provided policies:
Intravenous (IV) tubing- Changing of- Adults and Pediatrics (dated 9/2013) reflected, " ...2. Primary tubing a. Tubing may be used up to 96 hours.6. Date the tubing with the day the tubing is hung. A. Use of labels with day of the week will require adding a date ...."
Central Venous Catheter (CVC) - care of adult patient (dated 3/26/2016) reflected, " ...Definitions ...Implanted Port- CVC that includes a housing with a septum surgically implanted under the skin, attached to a catheter with the tip of the catheter in superior vena cava. Key points- Change transparent dressing every 7 days (including chlorhexidine sponge dressing and securement device) and PRN when dressing becomes wet, soiled or no longer intact. Change gauze dressing every 48 hour and PRN ...24. Date the dressing ..."

b.) During an interview on the morning of 2/26/18, Staff #8, Chief Nursing Officer (CNO) stated, "We try to bath them every day .... we ask the patients .... they document if they refuse ...."

During an interview on the morning of 2/26/18, when asked if she was offered a shower, Patient #4 stated, "I had to ask for it (the shower)."

During an interview on the morning of 2/26/18, on the facility's 3rd floor cardiac unit, when asked if she had been offered a shower, Patient #2 stated, " ...I've been here a week ...I haven't taken a shower, I had to wash myself with towels ...."

During an interview on the morning of 2/26/18, on the facility's 3rd floor cardiac unit, when asked if she had been offered a shower, Patient #3 stated in part that he had been at the hospital three days and had not showered or shaved during that time. When asked if he had been offered a shower or had asked for a shower, Patient #3 stated "No."

During an interview on the morning of 2/26/18, in the conference room, when asked for the policy on showers and linen changes, Staff #6, Quality and Improvement Specialist stated, "We don't have a policy."
