PREMISES ARE CLEAN AND ORDERLY

Based on observation, record review and staff interviews, the facility failed to maintain a clean and orderly premise and ensure proper maintenance of the hospital in the Emergency Department.

Findings included:

During a tour of the hospital on 9/9/2024 at 10:35 am with the Facilities Management Director Staff ID #55 and ED Nursing Director Staff ID # 56 the following observations were made:

Emergency Department Treatment Area:
* Peeling paint and brown discolored wall, at the junction of the wall and toilet in the patient restroom adjacent to triage
precluding the toilet and wall from being adequately cleaned.

* Dead bugs and dust in the lights outside ED rooms 1&2.

* Multiple water leak stains were observed in eight ED ceiling tiles throughout the
ED treatment area.

* Dust over the headwalls, on top of suction canisters and portable suction
located in ED patient Room #1.

Record review of facility policy titled "Cleaning Policy", issued 6/1/91, no review date listed, stated "Policy To present aesthetic, clean contamination free environment for patients, visitors and personnel."

Interview with Facility Director Staff ID # 55 on 9/9/24 at 11:00am. He stated that the facility has a part-time maintenance employee Staff ID #61 who is responsible for sheetrock repairs. He reported that Environmental Services employed staff were responsible for deep cleaning and dusting with room turnover between patients Monday-Friday until 330 pm while on-site. He reported after 330 pm and on weekends, that ED clinical staff retained responsibility for cleaning prn and between patients. He stated that Housekeeping supervisor staff ID # 64 lived close to the facility and was available to come in, if needed for cleaning. He stated she had not been called in for the past 1 year that he was aware of. He confirmed surveyor findings while touring the ED.
