Based on review of 1 of 2 clinical records, as part of a complaint investigation, there was a failure to to supervise patient care by failing to ensure assistance in maintaining the patient's hygiene needs.
Findings:
Citing patient #2, a 13 year old male with a diagnosis of mood disorder. Admission: 1/12/2010; Discharge: 1/17/2010. At time of admission the patient's mother reported the patient was mentally retarded and did not independently keep up with his hygiene; "he had to be forced." However, review of the Patient Observation Sheet noted daily throughout hospitalization the patient was "Independent" in caring for his hygiene. There was no documentation in the record the patient had had a bath throughout hospitalization.
