Based on observation, interview, and record review, it was determined that the facility did not meet the Condition of Participation (CoP) in Patient Rights by failing to:

1. Ensure Patient 1 who removed her toe nails due to anxiety "self-injurious behavior" had increased supervision. Patient 1 was identified as a danger to self per the 5150 (hold for evaluation) due to danger to self. (Refer to A 144)

2. Ensure the Unit Safety checks of the environment where conducted. (Refer to A 144)

The cumulative effect of these systemic problems resulted in the hospital's inability to provide quality health care in a safe environment.
