Based on observation and interview, it was determined the facility failed to maintain one heating, cooling and ventilation (HVAC) shaft enclosure affecting one of four floors in this component.
Findings include:
1. Observation on March 29, 2017, at 10:23 AM, revealed the HVAC shaft enclosure, located in the third floor Special Procedures Unit storage room had an unprotected penetration, seen from inside the shaft looking to the right above the access door.
Exit interview with facilities director and facility maintenance representative #1 on March 29, 2017, between 1:15 PM and 1:30 PM, confirmed the unprotected penetration in the third floor HVAC shaft enclosure.

Based on observation and interview, it was determined the facility failed to maintain one set of smoke barrier doors affecting one of four floors in this component.
Findings include:
1. Observation on March 29, 2017, at 10:25 PM, revealed the double set of smoke barrier doors, located outside the third floor Special Procedures Unit (SPU), had a gap between the door meeting edges that exceeded the maximum allowable gap of 3/16 inch.
Exit interview with facilities director and facility maintenance representative #1 on March 29, 2017, between 1:15 PM and 1:30 PM, confirmed the gap between the SPU smoke barrier doors was too large to resist the passage of smoke.
