Based on observation the facility light switch to med gas needs to be located 5'-0" above finish floor.

The inspector observed while accompanied by the Administrator and the Maintenance Director during the hours of the inspection from 2:00 pm to 4:00 pm that the facility failed to provide a history of records for receptacle testing per NFPA 99: 3-3.3.3. in patient care areas.
