Based on visual observation the facility failed to provide the continuation of the exit discharge to include access to the public way from all required exits. The access provides an easier transition for occupants to evacuate from all exits in the building.
The exit discharge was deficient for 1 of 2 exits.

Findings:

During the facility tour, between the hours of 4:00 PM and 5::00 PM it was observed that on the Mezzanine rear corridor facility had 14 beds that were that were unattended longer than 1/2 Hour making the Exit Access not accessible at all times.

Exit and directional signs are displayed with continuous illumination also served by emergency lighting system.

During the facility tour on 11-8-12 between the hours of 8:00 AM and 5:30 PM it was observed that on top of second floor roof there is 1 Exit Light outside of stair well that does not have continuous illumination.

Based on visual observation the facility failed to assure that the fire alarm system was inspected and tested in accordance with the Life Safety Code and NFPA 72. The fire alarm system gives a sense of security to offer an advance warning in fire and/or smoke emergency.
