Based on medical record review, the medical staff were not accountable for the quality of care provided to Patient #1, specifically for failing to accurately address the leg fracture.
Findings include:
-Review of the EMS Record dated 08/09/12 revealed that the 12 year old was playing football and during a tackle another player fell on his left lower leg, Patient #1 stated he heard and felt a "snap."
-Review of the Emergency Physician Record dated 8/9/12 revealed another kid fell on his left leg during a football game and complains of pain below the left knee, and it hurts to bear weight. There was a popping sensation. Physical exam revealed left leg is tender with swelling and ecchymosis. "X-rays revealed no acute fracture of the right leg." Given Tylenol & Codeine Elixir for pain at 09:20 pm. Still c/o pain. + ecchymosis medial left leg.
ADDENDUM: Called by Radiology tech. X-ray of L leg put in later after patient's parents had been told no fracture. X-ray of left tibia/fibula revealed displaced fractures.
-Review of the Discharge Instruction Summary dated 8/9/12 at 21:25 revealed Patient #1 had a sprained knee and was given crutches and discharged home.
There was no evidence that Patient #1's left leg fracture was addressed by the physician prior to discharge. The initial physician progress note addressed the patient's right leg.
