Based upon observation during the survey walk-through, exit signs are not placed to provide clear identification for access to the building exits. Failure to clearly define exit paths can result in confusion that can prevent or delay occupants from reaching the safety of an exit from the building in the event of fire conditions.
Findings include:
On 7/13/16 at various times between 8:45am and 10:00am during the survey walk-thru while accompanied by the DPO & MS it was observed that an exit sign near the nurse stations for each unit on the 1st, 2nd, & 3rd floors were provided which could misdirect occupants. Conditions observed include:
1. The double-sided directional exit signs at the east side of the nurse stations appear to direct occupants to the Med room door when viewed from the east side of the sign.
2. The west side of this sign is also partially visible from the main north-south corridor which can draw occupants from the main corridor where exit access is provided into the deadend portion of the corridor serving the Nurse Office, Showers and defined suite of sleeping rooms(2nd & 3rd floors only). This side of the sign is also directional to indicate the door at the Med room as a possible exit access door.

Based upon observations during the survey walk-through, document review, and staff interview, the facility is not in compliance with a series of Life Safety and other Code requirements that are not documented under other K-Tags.
Findings include:
Due to the number, variety, and severity of the life safety deficiencies observed during the survey walk-through, the provider shall institute appropriate Interim Life Safety Measures (ILSM) until all cited deficiencies are corrected. The provider shall include, as an attachment to its Plan of Correction (PoC) and referenced therein, a detailed narrative and proposed schedule for all such measures. The narrative shall describe all measures to be implemented, as well as the frequency with which they are to be conducted, and shall indicate the manner in which the measures are to be documented. The narrative shall also include comments related to changes in the ILSM to remain in place as work toward the completion of its PoC progresses.

Based upon observations and staff interview during the survey walk-through, the essential electrical system is not arranged in compliance with applicable Code requirements. Failure to install and maintain the essential electrical system with requirements can result in failure of the system to perform as intended during loss of normal electrical power.
Findings include:
On 7/13/16 at 9:30am it was observed during the survey walk-thru while in the company of the MS that the Life Safety branch panel of the emergency generator system serves equipment loads for the "Elevator oil cooler" which is an electrical load not permitted by NFPA 99-1999, 3-4.2.2.2(b) to be on the Life Safety branch.
