Based on observation and interview it was determined the facility stored three soiled linen bags on the floor of the soiled linen room on the patient room unit and two trash bags on the soiled utility room floor in the Emergency Department. The failed practice had the potential to affect the health and safety of all patients, staff and visitors due to the potential spread of contaminated materials from the floor of the soiled linen room to other parts of the building. The findings follow:
A. On a tour of facility with the Assistant Administrator and Maintenance Director on 09/04/13 at 1315, three full soiled linen bags labeled "biohazardous" were observed on the floor of the soiled linen room located on the patient room unit.
B. On a tour of the Emergency Department on 09/04/13 at 1330, two trash bags were observed on the floor of the soiled utility room in the Emergency Department.
C. The Assistant Administrator and Maintenance Director verified the bags were located on the floor when the condition was observed.

Based on observation and interview it was determined the facility did not maintain a clean and safe environment in five (#111, #103, #101, #118 and #119) of eight ( #111, #109, #103, #101, #114, #117, #118 and #119) patient rooms observed due to dust accumulation and two (#111 and #101) of eight ( #111, #109, #103, #101, #114, #117, #118 and #119) patient rooms free of insects. The failed practices had the potential to affect the health and safety of all patients, staff and visitors due to the health hazards associated with those conditions. The findings follow:
A. On a tour of facility with the Assistant Administrator and Maintenance Director on 09/04/13 at 1300 the following observations were made:
1) In Patient Room #111, an accumulation of dust was observed on top of the patient headwall light fixture. A dead cricket was on the floor near the air conditioning unit.
2) In Patient Room #103, an accumulation of dust was observed on top of the patient headwall light fixture.
3) In Patient Room #101, an accumulation of dust was observed on top of the patient wardrobe. A dead cricket was on the floor in the patient toilet room.
4) In Patient Room #118, an accumulation of dust was observed on top of the patient headwall light fixture and wardrobe.
5) In Patient Room #119, an accumulation of dust was observed on top of the patient headwall light fixture.
B. The Assistant Administrator and Maintenance Director verified the above conditions as they were observed during the tour.
