Based on observation, interview and record review, the facility failed to maintain the Air Conditioning HVAC system. All essential equipment and infrastructure of the facility must be maintained and working as designed within its usable life expectancy.

Findings include:

Observation, interview and record review with the plant operations staff, the facilities staff and administrative staff on Aug 4, 2022 between the hours of 10:00 AM to 12:00 PM on the 4th floor revealed the Air conditioning system (HVAC) is not working to its designed capacity. Additionally, the Air Conditioning System Dates back to 1987 and appears it has reached the end of its usable life expectancy. An assessment and certification from a certified mechanical engineer are due at this time.
