Based on review of reports and records, it was determined that the facility failed to vary the times for the required number of fire drills for each shift in the last 12-month period in accordance with NFPA 101 LSC (00) Section 19.7.1.2. This deficient practice could affect how staff react in the event of a fire. Improper reaction by staff would affect the safety of 1 patient, visitors and staff.

Findings include:

On facility tour between 9:30 AM and 1:30 PM on 5/15/2012, a review of the available fire drill reports revealed that the facility's Day-shift fire drills in 2012 and 2011 were conducted between the hours of 8:00 AM, 9:05 AM, 9:07 AM, and 9:00 AM not at varied times as required by Section 19.7.1.2.

Minnesota State Fire Code (07) Edition Section 312.1. Requires fuel tanks be protected with guard posts from vehicle impact. This deficient practice could affect all patients, visitors, and staff in the event of a collision.

Findings include:

On facility tour between 9:30 AM and 1:30 PM on 5/15/2012, it was observed that two vehicles were parked within 5ft. of the generator's 600 gallon fuel tank with no guard post protection as required by MN State Fire Code section 312.1.
