Based on clinical record review and interview, it was determined the facility failed to assure five (Patient #2-#5 and #7) of nine (Patient # 1-#9) clinical records reviewed of patients in isolation had documentation of the care provided related to isolation initiation, continuation or discontinuation of contact precautions. The facility could not assure care needs would be met related to the requirement for isolation procedures. The failed practice affected Patient #2-#5 and #7 and had the potential to affect any patent admitted to the facility with isolation needs. The findings follow:
Review of clinical records on 04/04/12 for patients #1- #9 who were listed as being in Contact Precautions revealed Patients #2-#5 and #7 clinical records lacked documentation of care needs for the initiation or continuation of isolation interventions.
During record review and in an interview with the Regulatory Coordinator on 04/05/12 at 1130, the findings were confirmed.
